Appointments can be made on our website under the tab 'Book an appointment' or by calling our friendly reception staff. A doctor's referral is only required from our Veteran's Affairs patients (D904), and CDM (previously known as EPC) patients.

 
 
 

Payment Methods Accepted

Visa, Mastercard, EFTPOS and Cash are all accepted forms of payment. Payments are due at the time of service. An official receipt will be issued at time of payment. If you have private health insurance bring your card and we can claim your rebate on the spot.


Veterans Affairs

Our physiotherapists are approved DVA health care providers. All that is required to access this service, is a referral from your GP (D904), and your DVA Card.


Cancellation Policy

If you are unable to keep a scheduled appointment, PLEASE ADVISE US 24 HOURS IN ADVANCE BY TELEPHONE or you may be charged a 50% cancellation fee. This amount cannot be recorded on your insurance receipt.


Compensable/Third Party Payers

We do directly bill third party insurers and workers compensation parties provided we have received and verified evidence of your insurance coverage prior to your first appointment.


Chronic Disease Management (CDM) Plan
previously known as Enhanced Primary Care (EPC)

What is a Chronic Disease Management physiotherapy appointment? Do I qualify?

The CDM plan allows General Practitioners (GP) to organise the health care of patients with chronic conditions.  A GP can allocate a total of 5 sessions per person, per calendar year which Medicare will help fund. Health providers who can provide sessions include physiotherapists and dieticians.

How much do the CDM physiotherapy appointments cost?

If you have received a CDM plan from your GP, you can receive Medicare rebates when you have an appointment. PhysioSport can assist you with injury recovery, but our physiotherapy services are NOT covered completely by bulk billing schemes. This means you will have to pay the difference between the fee charged and the Medicare rebate.

Who can qualify for CDM?

Patients who have both a valid Medicare card and a chronic medical condition are eligible. A chronic medical condition is one that has been present for at least six months. Examples of chronic conditions can include asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions, and stroke. You must see a GP who must complete a GP management plan and team care arrangements for you.
